After downloading one of the forms below, email, fax, or mail them to NHCCF at the contact information on the form (or on the "Contact Us" link on the left bar).

NOTE:  NHCCF is affiliated with The National Christian Foundation (NCF), and some of the forms below reference NCF (as well as Fidelity).

• NHCCF Legacy Fund Application – to open a Legacy Fund and to provide Legacy Fund testamentary advisement

Wire Notification Form

• Wire Notification Form – to notify NHCCF that you are wiring money to your Fund at NHCCF

Giving Securities

• Letter of Authorization for Stock Transfers – to transfer stock on deposit in your brokerage account. If your shares are held at Computershare, please contact Computershare and request your shares in certificate form. Computershare will send you a certificate, and then use the Certificate Release Request and the Irrevocable Stock Power Form below to transfer your stock certificate to NHCCF.
  
  If you are transferring stock using your brokerage firm's forms, please use DTC Clearing Number 0226 for National Financial Services and the account number on this Letter of Authorization. Please also send in this Letter of Authorization to our foundation so we will know which fund should receive your contribution.
   
• Master Limited Partnership Stock – If you would like to transfer these types of securities please contact the contributions department at 419-873-4673 prior to the transfer.
   
• Letter of Authorization for Mutual Fund Transfers – to transfer mutual funds to NHCCF.  Select the institution you're transferring from:
  • Fidelity - this form may also be used to transfer mutual funds if they are held at a firm other than the ones listed. Check with your broker to make sure the mutual funds are transferable. If they are not transferable, contact us at 419-873-4673.

  • Vanguard – Use this prefilled form to transfer Vanguard Mutual Fund Shares to NHCCF. Please fill out pages 3-6 of the form and fax to New Hope Christian Community Foundation at 419.727.9921 or send it via regular mail. Once received, the required corporate documents will be completed and submitted to Vanguard. Please allow 2-3 weeks for processing.

• Certificate Release Request – to transfer stock certificate(s) in your physical possession
   
• Irrevocable Stock Power Form (Fidelity Form) – to allow NHCCF/NCF to facilitate the processing of your certificate(s) in the absence of your endorsement on the certificate(s)

Testamentary giving forms

• Testamentary Gift Information - explains the process for naming NHCCF as the beneficiary in your will/estate 
   
• Giving Fund Succession Plan Letter of Advisement - allows you to recommend to NHCCF where your Giving balance is distributed upon your death 
    

Grant recommendation forms

• Domestic Grant Recommendation Form – for grants to U.S.-based charities (though recommending grants is free and fast on our Donor Website)
